A Comprehensive Overview of Specific Assessment Results

In Measurement Based Care (MBC), tracking assessment results is crucial for understanding patient progress and the effectiveness of treatments programs. Greenspace’s Assessment Results Dashboard provides a comprehensive view of these metrics, helping admins and supervisors monitor and analyze patient outcomes.

In this dashboard, users can look up assessment-specific results and stats. This dashboard is filterable by assessment, and will return stats on that assessment only. For example, you can look up the # of assessments completed, reliable change rate, and symptom severity change for PCL-5 in seconds.

Overview

This dashboard is designed to give supervisors an in-depth look at assessment results, offering insights into patient progress, reliability of improvement, and recovery rates. By presenting this data in a visual and user-friendly format, the Assessment Results Dashboard enables healthcare providers to make informed decisions and optimize program or unit performance, or meet reporting requirements for specific patient conditions.

Assessment Responses

  • Total Assessment Responses — Displays the total number of assessment responses collected for this measure.

  • Total Patients Who Completed Assessment — Shows the number of unique patients who have completed at least one assessment of this measure.

  • Patients by Number of Assessments Completed — Pie chart illustrating the distribution of patients based on the number of assessments they have completed (of this measure).

Assessment-Specific Outcomes

  • Average Score by Response — This tile shows the average score across all patients when they completed their 1st, 2nd, or 3rd etc. assessment responses. The patient sample size represents the total number of patients that have completed 1st, 2nd, or 3rd etc. assessment responses.

  • Reliable Change — Pie chart representing the percentage of patients who demonstrated reliable improvement, deterioration, or no significant change between their baseline and most recent assessment. This tile will only show data for assessments that have a Reliable Change Index (RCI) defined. If there is no graph showing, it means you have selected an assessment that doesn’t have a Reliable Change Index defined, or there are no patients who completed that assessment yet.

  • Reliable Improvement by Response — This tile shows the percentage of patients who reliably improved at every incremental assessment response. This tile will only show data for assessments that have a Reliable Change Index (RCI) defined. If there is no graph showing, it means you have selected an assessment that doesn’t have a severity threshold defined, or there are no patients who completed that assessment yet.

  • Symptom Severity Change — This tile show the percentage of patients in each symptom category, at the baseline assessment, compared to the most recent assessment. This tile will only show data for assessments that have a severity thresholds defined: OCI, SPIN, HAI, GAD-7, PDSS, PHQ-9, PHQ-15, PCL-5, OASIS, PSC-17, and WSAS. If there is no graph showing, it means you have selected an assessment that doesn’t have a severity threshold defined, or there are no patients who completed that assessment yet.

  • Symptom Severity Flow — This tile shows how patients have moved between symptom severity categories between their first and most recent assessment. The left side shows which category the patient was in for their first assessment, and the right side shows which category they are in based on their most-recent assessment. This tile will only show data for assessments that have a severity thresholds defined: OCI, SPIN, HAI, GAD-7, PDSS, PHQ-9, PHQ-15, PCL-5, OASIS, PSC-17, and WSAS. If there is no graph showing, it means you have selected an assessment that doesn’t have a severity threshold defined, or there are no patients who completed that assessment yet.

Recovery Metrics

  • Recovery Rate — A two-step funnel showing patients with caseness in their first assessment and below caseness in their most recent assessment. Only patients with two or more completed assessments of the same measure are included. These tiles will only show data for assessments that have a caseness threshold defined. If there is no graph showing, it means you have selected an assessment that doesn’t have a severity threshold defined, or there are no patients who completed that assessment yet.

  • Recovery by Treatment Start Date — This tile shows the percentage of patients who recovered and not yet recovered for each treatment start month. Only patients with two or more completed assessments of the same measure are included. These tiles will only show data for assessments that have a caseness threshold defined. If there is no graph showing, it means you have selected an assessment that doesn’t have a severity threshold defined, or there are no patients who completed that assessment yet.

Patient Results Table

  • List of all patients with at least two completed assessments, detailing their scores, changes, and number of assessments completed.
  • Select the three dots tool in the top right corner of the table to download this list.

What is the purpose of the Assessment Results Dashboard?

The Assessment Results Dashboard provides a detailed view of patient assessment outcomes, helping clinical supervisors track progress, identify areas for improvement, and make informed program decisions.

What assessments are included in the dashboard?

The dashboard includes all assessments that your organization uses, but certain outcomes tiles will only display data if that information is available. For example, Reliable Change and Caseness Thresholds need to be defined for the Reliable Change and the Recovery Rate tiles to work, respectively.

Can I customize the dashboard?

No, unfortunately we are not able to customize this dashboard. However, as it includes all assessments that Greenspace uses, it should give you any assessment you’d like to look up stats for.
