Access Points for the Intake List
There are three distinct ways (Access Points) a patient can appear on the Intake List:
- Email invitation from an Admin or Clinician.
- Referral from a third-party such as a Primary Care Provider, Hospital, or Community Agency
- Patient Self Referral
All Access Points allow an organization to collect relevant patient demographic information that can assist with the Intake List management.
Email Invitation from an Admin or Clinician
When adding a patient to Greenspace, a clinician or admin can assign the Intake tag to add that patient to the Intake list.
This Access Point can be beneficial for organizations that have dedicated resources to receiving patient referrals. Once an individual has connected with the organization, an admin or intake worker can invite them to Greenspace over email, have the individual sign up, and then assign measures to gather baseline clinical data for triaging patients and managing waitlists.
Once the client has been created and assigned the Intake Tag, they will appear on the Intake List.
Referral from a third-party such as a Primary Care Provider, Hospital, or Community Agency
Third-parties (PCPs, Hospitals, Community Agencies) can utilize a secure digital patient referral form to refer a patient. This Access Point replaces the need for email and fax system referrals, and provides community partners with a quick, efficient, and safe way to refer a patient to an organization.
The PCP, Hospital Admin, or Community Agency worker will be prompted to fill out a Patient Referral Form that the organization has enabled and customized.
Once completed, the patient will be added to the organization’s intake list for a clinic admin to identify, review, and triage.
The referring partner will receive a message of a successful submission, and be redirected back to the referral page.
Patient Self Referral
Organizations can configure the Intake module for patients to self refer directly through a personalized web portal. This allows patients direct access to your intake list and is an effective way to minimize administration time.
These self-referral patients will complete your Intake process and are able to complete assessments that the organization assigns.
