Viewing the Intake List
The Intake List presents a summary of all clients that need to be assigned to a program or provider. It allows intake workers and admins to easily and quickly identify and triage new clients.
The list is sorted into columns, which can be edited and modified at your discretion. For the example below, we can see the Date Created, whether they have been matched with a Provider, Suicidality, whether they have completed the Intake Assessment, and any available individual assessment scores.
How Patients Appear on the Intake List
For new clients, there are three access points to the Intake Module:
- By Invitation
- Independent Client Sign Up
- Referral by a healthcare provider.
All clients will be automatically assigned an Intake tag and be added on the Intake list to be triaged.
Customizing the Intake List
The Intake List can be customized to help you and your clinic triage patients effectively based on the measures that are important to you. This can give you a clear understanding of what patients need service sooner than others.
Using the ⚙️ icon, you can add or remove columns to your Intake List.
Some examples include Assigned Providers, Suicidality Risk, Normalized scores for Assessments and so on.
Each item on the list can then be sorted for high and low scores, as well as created date or last completed assessment.
How Patients are Tagged on the Intake List
When patients are created through the different Access Points, the Intake Tag allows for these clients to be presented on the Intake List.
When patients are created through Self Referral & 3rd Party Referral, the Intake tag is automatically assigned.
When patients are created via invitation from Greenspace, you will need to add the Intake tag as part of their profile creation to enable their view on the Intake List.
Additionally, when you select assessments in the intake category, it will automatically create an Intake tag.
When is the intake tag removed?
- When you assign a provider & remove the client from the Intake List
- When you remove the intake tag manually (and the patient will move to the Patient List)
