Intake Assessment

Greenspace offers a standard Intake Assessment which can be used to understand a patient’s current mental health over several categories. The Intake Assessment covers 4 main sections:

• Clinical Symptoms (Depression & Generalized Anxiety Disorder)
• Functioning
• Alcohol and Substance Use
• Additional Patient Information

Clients can complete this upon being added to your intake list, as well as other time points while they await clinical intervention.

Once completed, the Intake Assessment results will be available under the patient’s Summary Reports.
